With your application, please submit the documents and artifacts outlined below. Note that we are not asking you to create anything new, but simply attach examples of instructional plans from this year. Please upload each file using the following naming convention: Last Name_First Name_Name of Artifact

  • Resume

  • Unit Assessment: Your unit assessment should be an example of a high-quality unit assessment that you have used to backwards-plan instruction. It should represent your vision for what students should be able to know and do by the end of the unit. We prefer to see an assessment from your curriculum, although it is fine if you have made modifications. Please be sure to include an answer key or grading criteria.

  • Unit Plan: Please submit a unit plan aligned to the unit assessment. It should include the objectives you covered each of the days of instruction. It may be based on your network pacing guide.

  • Lesson Plan & Materials: Please submit the materials for a lesson that you feel is representative of your instruction. You should include the lesson plan (teacher-facing), as well as any student materials. The student materials may come from your curriculum, but we hope to see any materials or resources used to support instruction (such as guided notes, slide decks, formative assessments, or homework assignments).
